Winter Jam (Logansport Carpool)

Our Heartland Youth is taking a Spring Break trip to Fort Wayne for Winter Jam!

The cost of Winter Jam is $15 per ticket. Each teen is responsible for the cost of entry ($15) to Winter Jam and any other things they may want to purchase (food, merch, etc...)

We are taking teens from the church to and from Fort Wayne for this night. This form is get an accurate amount of people who is attending this concert for carpooling

I understand that I will need to have my teen dropped off at Heartland Church - Logansport by 4pm to receive carpool

I understand that we will most likely not get back until approximately 1am in the morning. I understand that I will be there to pick up my teen(s) from the church

I understand that is for our youth-ages (6th grade to 12th grade). If someone is younger, they will not be permitted to join our carpool. If someone is older than these ages, they will be asked to be a volunteer and must complete a background check to attend with our Heartland Youth team

I give my permission for the teen I am registering to attend Winter Jam 2026. In the event that my child becomes ill, violent or uncontrollable for staff/volunteers, I will pick him/her up within two hours. Furthermore, I release Heartland Church members, employees for any and all liability that may occur. I ask that Heartland Church take any and all measures necessary to ensure my child's health and safety. If your child is involved in a medical emergency, Heartland Church will contact parent/guardian and if necessary, Heartland Church will take your child to the nearest hospital
